Request a Donor Card
There are a number of ways to become an organ and tissue donor but the best is to sign up as a donor when you apply for or renew your driver's license. It is an excellent way to make your wishes known and to have them respected after you die as it will include you in your state's Donor Registry - which is legal consent for donation. The following New England states have DMV-based donor registries: Connecticut, Maine, Massachusetts and Rhode Island.
(NOTE: If you have a Connecticut driver's
license, you can now change your donor
designation in between renewals
with a form provided online. Here is a direct link
to that
form from
the CT DMV. This is a better option than a donor
card. We are working with other DMVs in the region to
offer this same option."
If you wishe to indicate your consent to donate using a donor card,
please fill out our request
form and we will mail a card to you. When
you receive your donor card, fill it out, sign it,
and
keep it
with
you.
