The Connecticut Donor Registy provides a way for Connecticut Drivers License and Identification Card holders to consent to organ and tissue donation when they apply for or renew their drivers license or ID card. The information is then made available to designated organ and tissue recovery organizations so that donation can occur after death and in fulfillment of the individual's desire to donate and to help others.

In Connecticut, you can sign up to be a donor in between renewal periods by simply filling out a form you can download and print right at home. Here is a direct link to that form from the CT DMV. If you do not have the donor designation on your drivers license or ID card, just fill out the form and mail it in to the DMV and you will be added to the Connecticut Donor Registry.
