Order your Donate Life License Plate

The Massachusetts Registry of Motor Vehicles has authorized the production of a Donate Life license plate for the Commonwealth as soon as 1,500 plate applications are received. To be one of the first to receive a plate, print out the attached .pdf document and mail it to us along with the required $40 check made out to the MA Registry of Motor Vehicles (please complete a separate application and send a separate check for each plate you would like to purchase).

Once the 1,500 mark is reached, the checks and applications will be forwarded to the Massachusetts RMV and the plates will be put into final production.

This is a great opportunity to spread the life-saving message of organ and tissue donation.
